Please make sure that ALL the required information is filled in. Once the form is completed, please click the ORDER button at the bottom of the form. This is a Wet Signature Required order process. After you submit your initial order form you will receive a PDF signature page which must be printed, signed with ink, and then either fax or mail the form back to us for final processing. Your credit report will then be mailed via certified mail or FedEx (if requested) to either business' mailing address or credit card billing address (whichever is designated below) within 24 hours of us receiving and successfully processing your request, and would be used solely for the purposes of obtaining and compiling your Commercial Credit Report. You will not be contacted by either our employees or anyone else, unless specifically requested on this form, for any reason, other than for the purpose of processing this order.

Please note that you have to be an owner of the company you are requesting a report on, or a corporate officer, designated and duly authorized by the entity and its board of directors to be able to request such information on or about said corporation.
